• Application for Determination of Civil Indigent Status (PDF) (provided by the Office of the State Courts Administrator)
  This form is found on the Florida Supreme Court’s website, and is used to determine if persons qualify to have the filing fee waived on a civil case or to enter into a payment contract for costs incurred in a civil case.
• Application for Criminal Indigent Status (PDF)
  This form is used to determine if persons qualify for the services of the Public Defender in criminal cases. There is a $50 application fee for each form completed and reviewed.
• Payment Plan Agreement (PDF)
  This agreement is used to establish a payment plan for fines/costs in felony, misdemeanor, criminal traffic, and civil traffic cases. There is a $25 administrative fee for each contract signed.
• Payoff Request Form (PDF)
  This form is used to request the total payoff due for any outstanding balances owed on criminal and traffic cases.
