• Application for Criminal Indigent Status (PDF)
  This form is used to determine if persons qualify for the services of the Public Defender in criminal cases. There is a $50 application fee for each form completed and reviewed.
• Change of Address Form (PDF)
  This form is used by persons wishing to change their address found in court files at the Clerk’s Office.
• Change of Address Form for Cash Bond Depositor (PDF)
  This form is used by persons who posted a cash bond and wish to change their address found in the court files at the Clerk’s Office. This form must be notarized and submitted to the Clerk’s Office.
• Court Records Search Request Form (PDF)
  This document is used to request a search of Lake County court records. Complete this form and email, mail, or bring it into our office. A payment of $2 per name, per calendar year to be searched must be received before the search will be completed. This search will not disclose records that have been sealed or expunged. Allow seven to ten working days for return of results.
• Criminal Election Affidavit Form 318: Circuit Criminal (PDF)  | County Criminal (PDF)
  This form is used for pleading “No Contest” to certain offenses under Chapter 318, Florida Statutes.

• Notice of Confidential Information within Court Filings: Circuit Criminal (PDF)  | County Criminal (PDF)
  This form is used pursuant to Florida Rule of General Practice and Judicial Administration 2.420(d)(2). The filer of a court record at the time of filing shall use this notice to indicate whether any confidential information is included within the document being filed.
• Payment Plan Agreement (PDF)
  This application is used to establish a payment plan for fines/costs in felony, misdemeanor, criminal traffic, and civil traffic cases. There is a $25 administrative fee for each contract signed.
• Payoff Request Form (PDF)
  This form is used to request the total payoff due for any outstanding balances owed to the Clerk on criminal and traffic cases. There is a $7 service charge for each payoff statement issued.
