• Change of Address Form (PDF)
  Submit this form if you wish to change your address with the Traffic Division at the Clerk’s Office.
• Driving School Affidavit (PDF)
  This form is used if you wish to exercise the option of attending Driver Improvement School instead of having points assessed to your driving record.
• Payment Plan Agreement (PDF)
  This agreement is used to establish a payment plan for fines/costs in felony, misdemeanor, criminal traffic, and civil traffic cases. There is a $25 administrative fee for each contract signed.
• Plea of Not Guilty Form (PDF)
  Submit this form if you wish to plead not guilty to a civil traffic offense.
• Waiver for Proof of Registration or Insurance (PDF)
  This waiver is used if you find it impractical to provide proof of vehicle registration or proof of security (personal injury protection insurance) as required by law. There is no fee associated with the filing of this waiver.
