PaternityThe Lake County Clerk of the Circuit Court & Comptroller’s Office processes petitions to determine paternity and for related relief filed in the Circuit Court. A Petition to Determine Paternity and for Related Relief Form (PDF) (provided by the Office of the State Courts Administrator) can be used by a birth mother or father to ask the court to establish paternity, a time-sharing schedule, or child support of a minor child. The instructions provided with the form should be read and followed. In order to amend the paternity information for a child, a Certified Statement of Final Judgment of Paternity Form -DH673 (PDF) (provided by the Florida Department of Health) must be completed and submitted to the Lake County Clerk’s Office. The Lake County Clerk’s Office will forward a copy, along with any other necessary forms, to the Florida Department of Health, Bureau of Vital Statistics within 30 days after the final judgment of paternity. The Fifth Judicial Circuit provides checklists of items needed for paternity on its website. Other related forms can be found on the Office of the State Court Administrator’s website. Submission of DocumentsThe Florida Rules of General Practice and Judicial Administration require that petitions, pleadings, and documents be filed electronically except in certain circumstances. Self-represented litigants (pro se) may file petitions or other pleadings or documents electronically; however, they are not required to do so. If you choose to file your pleadings or other documents electronically, you must do so in accordance with Florida Rule of General Practice and Judicial Administration 2.525.
