  • Declaration of Domicile
    Used to establish a domicile in Florida for persons who want to evidence the same by a recorded document. It is primarily used by persons wanting to claim homestead exemption through the Property Appraiser’s Office. The recording fee is $10. If you need assistance from a clerk in completing the information on the form, there is a $5 fee for this service (per FS28.24(15)) as well as an additional fee of $10 if a clerk notarizes the form.

  • Official Records Order Form (PDF)
    Used by non-commercial entities only to request copies and certified copies of documents from the Official Records Books. After printing and legibly completing this form, it must be presented, delivered, or mailed to the Lake County Clerk of the Circuit Court, Recording Section, 550 W. Main St., P.O. Box 7800, Tavares, FL 32778-7800, along with a check or money order made payable to “Gary J. Cooney, Clerk of the Circuit Court & Comptroller.” Copies are $1 per page and $2 per document to certify (if certification is requested). There is also a postage and handling charge of $1.

  • Recording Transmittal Form (PDF)
    This form is included with documents submitted for recording into the Official Records of Lake County. Although not required, it assists in providing the Recording office with the information necessary to record documents accurately and promptly.

  • Release of Exempt Information for Title Search (PDF)
    This form is used by title companies and attorneys to request the release of exempt information in order to perform a title search. A Florida Company Code or License Number or Florida Bar Number is required. The form is recorded into the Official Records and affected party(ies) are notified of the request by the Clerk’s Office. Recording fees and notification fees are charged for this service.

  • Request for Confidentiality – Official Records (PDF)
    Used by qualifying individuals to request the redaction of personal information on documents that are maintained in the Lake County Official Records. The request must be legibly written, signed, and delivered to the Lake County Clerk of the Circuit Court & Comptroller’s Recording Section. The request must specify the identifying number (Book and Page or Clerk File Number) that contains the personal information to be removed. No fee is charged for this service.

    NOTICE OF THE RIGHT OF ANY AFFECTED PARTY TO REQUEST REMOVAL OF CERTAIN CHAPTER 119 INFORMATION OR RECORDS:

    Any person has a right to request that a county recorder remove from a publicly available Internet website information made exempt from inspection or copying under s. 119.071 or an image or copy of a public record, including an official record, if that image or copy is of a military discharge; death certificate; or a court file, record, or paper relating to matters or cases governed by the Florida Rules of Family Law, the Florida Rules of Juvenile Procedure, or the Florida Probate Rules. Information removed as exempt under s. 119.071 will not be removed from the Official Records as described in s. 28.222(2). Such requests must be made in writing and delivered in person, by mail, or by electronic transmission to the county recorder. The request must identify the Official Records book and page number, instrument number, or clerk’s file number for any information or document to be removed. For requests for removal from a person claiming a public records exemption pursuant to s. 119.071, the request must be written; be notarized; state under oath the statutory basis for removal of the information, image, or copy that is restricted from general public display on the county recorder’s publicly available Internet website; and confirm the individual’s eligibility for exempt status. A party making a false attestation is subject to the penalty of perjury under s. 837.012.

    Any person has a right to request that a county recorder or clerk of the court add information to a publicly available Internet website if that information involves the identity of a respondent against whom a final judgment for an injunction for the protection of a minor under s. 741.30, s. 784.046, or s. 784.0485 is entered, unless the respondent is a minor. Such requests must be made in writing and delivered by mail, facsimile, or electronic transmission or in person to the county recorder or clerk of the court. The request must specify the case number assigned to the final judgment for an injunction for the protection of a minor under s. 741.30, s. 784.046, or s. 784.0485.

  • Request Form for Release of Exempt Information (PDF)
    Used by qualifying individuals who have requested redaction of personal information in the Official Records to authorize the release of redacted information to specific parties. The request must be legibly written, signed, notarized and delivered to the Lake County Clerk of the Circuit Court & Comptroller’s Recording Section. The request must specify the identifying page number (Book and Page or Clerk File Number) to be released. No fee is charged for this service.

  • Request Form for Internet Image Removal (PDF)
    Used to request the removal of certain sensitive document types from the Official Records which are publicly available on the Clerk’s website. Any affected person may request that images of the following types of documents be removed from the Official Records which are publicly available on the Clerk’s website:

    • Military Discharges
    • Death Certificates
    • Documents relating to matters or cases governed by:
      • The Florida Rules of Family Law
      • The Florida Rules of Juvenile Procedure
      • The Florida Probate Rules

The request must be in writing and may be delivered in person, by mail, email, or by fax. The request must specify the identifying page number (Book and Page or Clerk File Number) of the document to be removed. No fee is charged for this service.

Florida law requires that images and copies of the above-listed documents remain on file and available to the public upon request in the Office of the Clerk or County Recorder.

  • Request Form for Social Security or Account Number Removal (PDF)
    Used to request the redaction of social security or account numbers on recorded documents that are publicly available on the Clerk’s Internet website. The request for removal must be legibly written, signed, and delivered in person or by mail, fax, or email to the Clerk’s Office. The request must specify the identifying page number (Book and Page or Clerk File Number) that contains the social security or account number. No fee is charged for this service.
