What if I don’t have a copy of my birth certificate?

A certified copy of birth records can be obtained by contacting the Health Department’s Bureau of Vital Statistics Office in the applicant’s state of birth. You can visit the Florida Department of Health’s website or call 800-255-2414 for additional information.
